Provider: Texas Municipal League Intergovernmental Employee Benefits Pool (TMLIEBP) using United Healthcare Choice Plus Network
Medical Insurance for employee only is covered at 100%.
100% participation plan, 90/60, $500 in-network deductible, $750 out of network deductible and $1,500 out of pocket, Mac C Prescription Plan (Medication Therapy Management Program).
Dependent Medical Coverage (spouse and/or children regardless of number) $1,125.90 - $220 paid by city = $905.90/month cost to employee.
Employee & Family Dental is covered at 100% .
$10,000 Basic Life and AD&D for employee only is covered at 100%.
Employee can purchase additional life insurance up to 3x salary with a maximum of $300,000 (cost varies by age ex: $270,000 for a 50 year old = $89.64/month, $270,000 for a 65 year old = $408.51/month.
Vision Coverage is available on a voluntary basis ($12.50 monthly cost to Employee or $ 25.00 monthly cost for Family).
The city does not offer dependent life insurance.
Retiree Medical Insurance is offered at 195% of actual cost with no contribution from the city.
Provider: Texas Municipal Retirement System (TMRS)
Participation required, 6% employee contribution, vested after 5 years, 2:1 city match, retirement at or after age 60 or at any age after 20 years of participation.
Accidental death and disability benefits are also incorporated into this retirement plan should an employee become disabled prior to retirement.
$25 for 1st year of employment, $25 each year thereafter up to $500 per year maximum.
10 days per year from the 1st through 5th year of employment.
1 additional day per year 6th year and ending with a maximum of 20 days the fifteenth year (Example: 6 years = 11 days per year, 7 years = 12 days per year, etc.).
12 days per year.
